A UFO sighting report form that never leaves your device.

Write it down while it is fresh. The form scores how useful your report will be to the person who checks it, saves a clean PDF, and shows you where to send it. It does not judge what you saw.

Fill in what you can.

Blank is better than a guess. Your draft saves in this browser as you type, and nothing on this page is uploaded.

Report strength: 0 of 100 · Empty · What raises it

Next: Add the date, start time and place.

Say what you saw, not what you think it was. An investigator can work with “a steady white light moving north for 40 seconds”. They cannot work with “a craft”.

01When

Local time where you were. Rough is fine.

The report adds the same moment in UTC, the time investigators compare against.

Count or estimate. “About 40 seconds” beats “a while”.

02Where

Town, road, park or landmark.

Your browser asks first. The numbers are written into this form and go nowhere else.

03Sky and weather
04What you saw

Hold your arm straight out: a fist covers about 10° of sky (EarthSky), your little finger about 1°, and the full Moon about half a degree (NASA Night Sky Network).

What happened, in order, from the moment you noticed it to the moment it was gone.

05Direction
Where it was when you first saw it
Which way it moved
06Other witnesses

One person across town who saw the same thing is worth more than three people on your porch, because they could not have talked each other into it.

07What you recorded
Recorded on a device

The original is the first file on the device that took it. A screenshot, a crop, or a copy sent through a messaging app loses the data inside the file.

Before you share a photo: it can carry the place and time it was taken inside the file. That helps an investigator, and it also tells anyone you send it to where you were. If you are under 18, show this to a parent or guardian before you send it anywhere.

08What you checked
Ordinary explanations you ruled in or out

One line is enough, for example: no aircraft on the flight tracker at 21:40. Ticked checks count toward the score once this is filled in and the date, time and place are in.

Not sure how to check? Run it through What did I just see? first, then press Log this sighting there.

Nothing on this form is uploaded.

Draft

Your report.

This is what prints. Choose Save as PDF in the print dialog for a one or two page file, or copy it as plain text and paste it into a reporting form.

The draft stays in this browser until you clear it. Drafts older than 30 days are cleared automatically.

Where to send it.

If anyone is in danger, call 911 or your local emergency number first. Otherwise, pick the place that fits. We do not fill in or send any of these forms for you.

Best fit for your report

  • Fill in the form above and this points you to the place that fits your report best.
  • AARO, the Pentagon office, does not take reports from the general public.
  • NUFORC

The National UFO Reporting Center, a civilian reporting center with an online report form and a public database of past reports.

    US and worldwide
  • MUFON

The Mutual UFO Network, a membership group of volunteer investigators. Reports go into its case management system.

    US and worldwide
  • Canadian UFO Survey

Run by Ufology Research, which has published the survey every year since 1989. Its form says witness names and contact details stay private and are left out of the survey.

    Canada
  • Sky Canada Project

The federal science office does not take reports. It says UAP reports are not collected systematically by the Canadian government or any other official organization, and to contact local authorities if something looks like an immediate hazard. Its June 2025 report explains the gap.

    Canada, official
  • American Meteor Society

For a fast, bright streak that was gone in seconds. That fits a fireball, a very bright meteor, and the AMS collects public fireball reports.

    Fast streaks
  • AARO

The Pentagon’s All-domain Anomaly Resolution Office does not take reports from the general public. Its reporting page is for current or former US government employees, service members and contractor personnel with direct knowledge of US government UAP programs or activities.

    Not for the public

Add your name and contact details only on the form you send it to. This page never asks for them. For what to do in the first minutes, read how to report a UFO sighting.

How the score is counted.

The meter adds up nine parts. It borrows its approach from the Rio Scale used for claimed SETI signals: judge the quality of the evidence, not the excitement of the claim.

PartPointsWhy it matters
Date and start time12Lets someone match your sighting against flights, satellite passes and rocket launches.
Place name and coordinates12The sky looks different from every spot on the ground.
How long it lasted8Separates a two-second meteor from a four-minute satellite pass.
Shape, color, brightness, size8Comparisons to known things can be checked. Adjectives cannot.
Where it was and which way it moved5A direction turns a light into a track someone can follow up.
Independent witnesses15People in other places who will confirm it are the strongest thing a civilian report can carry.
Original files kept15The original file holds the time, settings and often the place. Copies lose them. A ticked photo or video counts once you name the phone or camera.
Ordinary explanations checked15Three points for each of aircraft, satellites, planets, drones and lanterns, once the date, time and place are in and you write down what the checks showed.
Written within an hour10Notes made soon after are less likely to absorb what you heard or read later.

Bands: 0 to 24 thin, 25 to 49 workable, 50 to 74 solid, 75 to 100 strong. Until the date, start time and place are all in, the score stops at 24, because nobody can check a sighting without them. A strong report can still turn out to be a satellite. That is a good outcome: it means the report was clear enough to solve.

A clean report starts with a calm witness.
